There are some studies that have compared SRS platforms for treating multiple BM. Ma et al. reported that the dose delivered to normal brain is dependent on the radiosurgery platform: Gamma Knife (superior), linear accelerator (linac) or CyberKnife [10]. SRS plans for the treatment of multiple metastases with single-isocenter volumetric modulated arc (VMAT) resulted in comparable dose gradients in the high dose area, but the mean brain dose with VMAT was higher than with the other techniques [11]. Based on two cases involving three and seven BM, Eaton et al. compared treatment platforms of 21 clinical centers and concluded that most platforms were able to achieve comparable plans, except for the smallest volumes and when larger planning margins are used [12].
One would expect a better OARs sparing with separate-isocenter DCA plans because of a possibility to avoid the critical structures. According to ANOVA in our study, the maximum dose to brain stem, chiasm, optic nerves, and eyes were not statistically significantly different between the seven planning techniques. This result was not published before.
We strongly recommend that studies examining neuropsychological/neurocognitive functioning administer comprehensive neuropsychological batteries, in consultation with a Clinical Neuropsychologist, whenever possible, especially batteries that have been well-validated in HIV+ individuals, such as those used in the CNS HIV Antiretroviral Therapy Effects Research (CHARTER) and Multicenter AIDS Cohort Study (MACS) studies [20,75], and apply the best available normative data [76,77]. In clinical settings, comprehensive neuropsychological evaluations may not always be feasible, and in such cases, groups such as the Mind Exchange Working Group [78] have discussed the strengths and limitations of the available screening tools. Neurocognitive screening, at minimum, is now recommended for all HIV+ patients, regardless of reported symptoms [43,78]. It is important to note, however, that screening tools often lack the sensitivity to identify less severe forms of HAND and also lack the specificity of comprehensive, multi-domain batteries. Comprehensive neuropsychological evaluation remains the criterion standard in the diagnosis of HAND.

Descriptive, retrospective study, classifying independent variables in seven key dimensions: identification/risk rating; length of stay/use of advanced life support; imaging; clinical analysis; special procedures; blood products in the intensive care unit; and in the operating room. To describe measures that assaulted EMS personnel believe will help prevent violence against EMS personnel. This mixed- methods study includes a thematic analysis and directed content analysis of one survey question that asked the victims of workplace violence how the incident might have been prevented. Of 1778 survey respondents, 633 reported being assaulted in the previous 12 months; 203 of them believed the incident could have been prevented and 193 of them (95%) answered this question. Six themes were identified using Haddon's Matrix as a framework. The themes included: Human factors, including specialized training related to specific populations and de-escalation techniques as well as improved situational awareness; Equipment factors, such as restraint equipment and resources; and, Operational and environment factors, including advanced warning systems. Persons who could have prevented the violence were identified as police, self, other professionals, partners and dispatchers. Restraints and training were suggested as violence-prevention tools and methods CONCLUSIONS: This is the first international study from the perspective of victimized EMS personnel, to report on ways that violence could be prevented. Ambulance agencies should consider these suggestions and work with researchers to evaluate risks at the agency level and to develop, implement and test interventions to reduce the risks of violence against EMS personnel. These teams should work together to both form an evidence-base for prevention and to publish findings so that EMS medical directors, administrators and professionals around the world can learn from each experience. The San Francisco Fire Department's (SFFD; San Francisco, California USA) Homeless Outreach and Medical Emergency (HOME) Team is the United States' first Emergency Medical Services (EMS)-based outreach effort using a specially trained paramedic to redirect frequent users of EMS to other types of services. The effectiveness of this program at reducing repeat use of emergency services during the first seven months of the team's existence was examined. A retrospective analysis of EMS use frequency and demographic characteristics of frequent users was conducted. Clients that used emergency services at least four times per month from March 2004 through May 2005 were contacted for intervention. Patterns for each frequent user before and after intervention were analyzed. Changes in EMS use during the 15-month study interval was the primary outcome measurement. A total of 59 clients were included. The target population had a median age of 55.1 years and was 68% male. Additionally, 38.0% of the target population was homeless, 43.4% had no primary care, 88.9% had a substance abuse disorder at time of contact, and 83.0% had a history of psychiatric disorder. The HOME Team undertook 320 distinct contacts with 65 frequent users during the study period.
